Step by Step on How to Fill out Short Staffing Forms on the CHS Website (MHB & KMH):

1. Go to chrome to get on to the CHS home page. 
2. Scroll down and on the left hand side there is “NYS Staffing Form.” 2. 
3. Click on it. 
4. Click on bypass log in. 
5. Click on the 3 dots … to pull up hospital list. 
6. Click BMH short staffing v2. It is the first one OR KMH short staffing v2. It is the second one
7. Everything that is in bold needs to be filled out, date, etc. 
8. Please fill in your names
9. Fill out the description with everything that happened on that shift. Examples: you pulled the floor’s NAs/ITAs to sit in a 1:1 which is shorting the floor, charge took patients, no charge, # of RNs and aides, if you missed breaks, short imaging techs, etc. 
10. If you are using the buddy system to cover each other's breaks, AKA you are watching another staff member’s patients which is going over the contract ratio, please click the box that states covering another staff members assignment.
11. Fill out all applicable boxes, we need the numbers for data. (MERCY)
12. Fill out census that is in bold or put N/A. (KENMORE)
13. If you fill in the description box, you do not have to put anything in the specific boxes for RN, ITA, Techs, etc. (KENMORE)
14. Fill out manager/supervisor notified section, you can use the supervisor on shift that gave you staffing or management.
